The Monarch flights stops off in Grenada en route to Tobago but is a direct flight back.

Flights from the USA and Canada

Caribbean Airlines flies from most major gateways to Port of Spain in Trinidad and then on to Tobago.

Delta Airlines fly from Atlanta to Tobago.  This is the only direct service from North America to Tobago. They also fly to Trinidad from JFK New York.

Most major US airlines will get you to Trinidad from New York or Miami and Tobago is then only a short 15 minute flight away but you need to arrange that yourself and be careful to check times and availability.
Air Canada flies from Toronto to Port of Spain, Trinidad from where you can then arrange a flight over to Tobago.

Getting to Tobago from other Caribbean islands

Caribbean Airlines  flies from Port of Spain to Tobago 12 times daily from 06:00 to 21:15.
It costs $25 U.S. one way and the flight takes 20 minutes. Tickets can now be obtained through travel agents.
